Active Health Partners have been engaged by your Employer to facilitate your participation in completing our online Working From Home Checklist & WHS Training.
We respect your right to privacy and confidentiality. The information you provide to Active Health Partners (AHP) is securely stored on encrypted cloud-based servers and will not be shared with third parties, other than your Employer, without your consent.
By agreeing to the Terms & Conditions you are acknowledging that, in completing and submitting the Working From Home Checklist & WHS Training, all of your responses, including any uploaded photos, will be distributed to both you and your Employer.
You will be asked to provide some personal details and information related to your employment and working from home arrangement. You will also be asked some questions about your physical and mental health. You are not obligated to provide your employer with personal or medical in-confidence information, should you not wish to do so.
You will be asked to complete a Working From Home Checklist. This involves:
Completing an online checklist to help you identify potential safety hazards related to you working from home arrangement
Taking and submitting photos of your workspace including your workstation setup, seating and any potential safety hazards you’ve identified.
Notifying your employer about any potential safety hazards you’ve identified that you are unable to resolve, without assistance.
We advise that you remove any confidential or personal information from your workspace prior to completing this task and ensure that there are no people in any photos, including yourself.
